Diabetes Care CenterThe Diabetes Care Center of Community Health Network strives to provide individualized, quality education programs and services to all people with diabetes. Family members and significant others are encouraged to participate. The programs promote self-care behaviors for the management of diabetes in collaboration with the health care team so you may return to health quickly and focus on what’s important in your life.

With a team of health care professionals specialized and certified in diabetes, the center offers education programs and services to any person interested in learning more about diabetes self-care and management. Services are provided in both group and individual settings and are designed to meet various age groups, schedules and learning needs. Programs and services are offered at the Diabetes Care Center at Community Health Pavilion Fort Ben, at Community Hospital South and at Community Hospital Anderson.

Diabetes Education and Self-Management Program

Here, a team that includes a nurse practitioner, certified diabetes educators, registered dieticians, pharmacists and social workers help patients learn the tools and techniques to best manage their disease. In cooperation with each patient's primary care physician, the program also provides insulin and medication assessment and adjustment with frequent phone contact until blood glucose is at or near target levels.

This program consists of three three-hour group class sessions which are offered in the morning, afternoon and evening.

The program is certified by the American Diabetes Association (ADA).

Insulin Pump Program

After assessing whether a patient is an appropriate candidate for insulin pump therapy, a nurse practitioner and certified pump trainer will train qualified patients to use the pump, calculating such measures as carbohydrate ratio and insulin sensitivity to determine pump rates.

Gestational Diabetes

Certified by the American Diabetes Association (ADA), this program, which offers individual and group instruction, closely monitors patients' blood glucose control and provides free loaner glucose monitors to all patients. Meal plan and nutritional counseling are also part of this program.

Program staff coordinate care with each patient's obstetrician and/or primary care physician and contact patients within 24 hours of referral notice.
